All week our focus on Outlook has been health care - from men's health to the epidemic of obesity to navigating the maze of managed care. In our weekly roundtable discussion, we bring together physicians from different specialties to look at how - as consumers and patients - we can get the most from our health care. We begin where many of us begin - overcoming the fear of making a doctor's appointment because we're afraid of what we might hear once we get there.

1. A lot of people are afraid to go to the doctor because they maybe haven't been regularly, or they are afraid of hearing something they don't want to hear What would you say to them to get them in the door?
-the doctor as an authority figure -
-how would you like your patients to see you?
2. So after you make an appointment with a doctor, how can you make the most of the time you have together?
-make lists of questions or topics
-bring someone else with you
3. You've had symptoms for a couple of weeks - how do you know when it is time to call the doctor? What is the line between being proactive and maybe making too many calls to the doctor?
4. You make an appointment to see your doctor and when you arrive you are seeing the nurse practitioner or physician's assistant. Should you feel like your problem is not important enough to warrant the doctor's attention?
Is there ever a time when you should demand to see the doctor?
5. So you are referred to a surgeon. Dr Coppola, how should we be better prepared for that visit? Should I assume my records will be sent to you ahead of time? This is a stressful situation for many. There is a problem of some sort, that is why they are there. Does that level of anxiety in the patient make your job more difficult? Should we as patient bring someone with us as a "second set of ears?"
6. What about getting a second opinion? How and when should I ask for a second opinion? Some might be afraid of asking for that opinion.
7. The current health care system - the "business side of medicine" does it impact you in any negative ways - do you feel crunched for time.swamped with the necessary paperwork of managed care. Does the system ever compromise care?
8. Let's look down the road at the future - what changes might you forecast? Some practices are making use of websites etc.
what about the use of holistic approaches?
9. Is the reality of your practice of medicine different than what you imagined when you were in med school? What's the best part of being a doctor? What maybe is the down side?
